PILES (HEMORRHOIDS) TREATMENT
The varicose condition of the hemorrhoid plexus of veins situated in the lower portion of the rectum and the anal canal is called piles. Piles mean ball-like mass, present in the anal region. The condition where blood oozes rectally that condition is called piles.

Occurs between the primary hemorrhoids
1st degree haemorrhoids
Only bleeding, Piles mass does not comes out.

4th degree haemorrhoids

TYPES OF PILES LOOKS LIKE
FEATURES
- Bleeding-Splash in the pan, bright red and fresh blood during deification.
- Prolapes of piles mass during defication.
- Pain-usually painless. in case of thrombosis, infection, strangulation or associated with fissure in ano, it will be so painful.
- Mucous discharge.
- Anemia.
- Discomfort during sitting posture.
CAUSES
- 1. Hereditary-Congenital weakness of the veins wall
- 2. Anatomical-absence of the valve in superior heamorrhoidal plexus causes high pressure.

FISSURE IN ANO TREATMENT
A fissure is an ulcer in the longitudinal axis of the lower anal canal. Commonly it occurred in the midline posteriorly but can also occur in the midline anteriorly. Fissure in ano on the posterior midline is more common in a male patient whereas fissure in the anterior midline is more common in female patients.
In the upper end of the fissure, there is frequently a hypertrophic anal papilla and in its lower end, there is a hanging of hypertrophic skin which is called a sentinel tag.
TYPES
ACUTE FISSURE
Symptoms:
1. It is a deep tear in the lower anal skin with severe sphincter spasm, without oedema and inflammation.
2. Presents with severe pain and constipation.
CHRONIC FISSURE
Symptoms:
1. It has an inflamed, indurated margin with scar tissue.
2. Ulcer at its inferior margin is having a skin tag, which oedematous acts like a guard. A chronic fissure is less painful than an acute one

FEATURES
- It is common in middle-aged women not in the elderly.
- Severe pain during and after defecation.
- Streaks of fresh blood with stool.
- Constipation.
- Lack of digestion.
CAUSES

1. Because of the curvature of the sacrum and the rectum. Hard faecal matter while passing down causes a tear in the valve leading to the posterior midline.

3. Hard stool, diarrhoea, increased sphincter tone, trauma, sexually transmitted diseases.

Fistula in ano is an inflammatory tract that has an external opening in the perianal skin and an internal opening in the anal canal or rectum. CAUSES
PRIMARY CAUSE
The fistula usually originates from a perianal abscess in the intersphincteric space of the anal canal from infection of the anal gland

CLINICAL FEATURE
- 1. Persistent seropurulent discharge keeps the part always wet.
- 2. Previous history of anal gland infection with recurrent abscess.
- 3. The external opening can be single/multiple pouting granuation tissues and may discharge blood.
- 4. Ischiorectal fossa on each side, most open communicates with each other behind the anus causing horseshoe fistula.
- 5. Per rectal examination shows indurated internal opening usually in the midline posteriorly.
- 6. Tubercular fistulas does not have induration will have pale granulation tissue.
- 7. Internal opening in carcinoma felt as a buttonhole defect inside the rectum.

- 2. Low anal- common below anal ring
- 3. Submucous- above the dentate line
- 4. High anal- on or above the anal ring
- 5. Pelvi rectal- situated between the upper surface of the leveterani and pelvic peritoneum.
